GROUP OR INDIVIDUAL INSURANCE
If you are unsure of your insurance benefits, we can call your insurance company to verify your type of coverage. Benefits quoted by your insurance company do not guarantee payment of services. Upon receipt of payment from your insurance company, please remember that you will still be responsible for any non-covered services, deductibles or co-pays. If you have any questions regarding you financial responsibilities, please feel free to ask. Our staff will be happy to assist you.
PATIENTS WITHOUT INSURANCE
Although having health insurance is ideal, not having health benefits or having limited coverage, i.e. catastrophic policies, is a reality. With that in mind, Dr. Lombardi has several options for patients in order to receive complete care without the burden financial difficulty. Please address any financial concerns with the members of our staff and remember, not receiving treatment should always be the last course of action.
Should you be injured “on the job”, depending on your health insurance provided by your employer, your care should be paid for under your employer’s Worker’s Compensation insurance. In order to be treated under worker’s compensation, you must inform your employer of the accident and obtain the name and address of their insurance carrier. If your employer does not provide this information, or a settlement has not been made within 3 months, or you are suspend or terminate care, any service fees will be due immediately.
PERSONAL INJURY OR AUTOMOBILE ACCIDENTS
Dr. Lombardi accepts personal injury cases as well as auto insurance claims. In the case of personal injury or automobile accidents, we will suspend your bill while waiting for settlement of your claim for a time period after services are provided, of which will be negotiated with our billing staff. Notification to your auto insurance carrier upon your receipt of services must be done immediately. In the event an attorney will be representing you, please notify our billing staff immediately. Once your claim is settled or you are suspend or terminate care, any service fees will be due immediately.
In the event where secondary insurance will be filed, please inform our billing staff. We will be happy to assist you should you require it.
